Accuray Cyberknife Xchange Robotic Collimator Changer
Accuray is recalling the Xchange Robotic Collimator Changer for the Cyberknife system due to a risk of the collimator falling from its housing.
Immediately cease using the in-room control pendant to initiate motion if a system interlock has occurred. Contact Accuray Incorporated for instructions on system inspection and software updates to resolve the docking failure.
Why it was recalled
Accuray is recalling the Xchange Robotic Collimator Changer used with the Cyberknife Treatment Delivery System. A failure in the docking mechanism can cause the collimator to release and fall from its housing if motion is initiated via the in-room control pendant after a system interlock occurs.
