What happened
Fagron Compounding Services of Wichita, Kansas, began a recall on March 20, 2026, for Vancomycin HCI and norepinephrine Bitartrate injections because of a lack of assurance of sterility and because a blue Break-Off-Part could detach from the administration port.
The affected products include Vancomycin HCI in 1.5 gram, 1.25 gram, and 1 gram strengths added to 250 mL of 0.9% Sodium Chloride Injection USP, and norepinephrine Bitartrate in 32mg and 16mg per 250mL strengths. The products were distributed nationwide in the U.S.A. The FDA added the recall to its Enforcement Report and classified it on April 7, 2026.
