FDA recall, , Moderate risk

Da Vinci X, Xi, and 5 Surgical Systems

The da Vinci surgical systems have a software error that can allow failing instrument arms to remain in use.

What happened

Intuitive Surgical, Inc., of Sunnyvale, California, recalled its da Vinci X, Xi Surgical System, and da Vinci 5 Surgical System because a software implementation error allowed instrument arms failing an important diagnostic test to remain in clinical use.

The test was supposed to be used to detect Universal Surgical Manipulator insertion axis ball screws that exhibit the symptoms of being bent and misaligned, a precursor to potential ball screw fatigue failure. The recall began November 7, 2025, and was added to the FDA's Enforcement Report on February 2, 2026.
