What happened
Luxfer Canada Ltd. (Luxfer) is recalling certain Unified Fine Pitch thread adapters, model EP-O-350-8-U5. The O-ring recess in the gas cylinder adapter may be incompatible with certain valve types. Using an incompatible valve may reduce the O-ring compression, increasing the risk of a hydrogen leak and fire.
The recall affects 83 units distributed nationwide. The issue was reported to NHTSA on March 27, 2026. Owner notification letters were mailed April 1, 2026.
Luxfer will provide the customer with a replacement UFP adaptor, free of charge.
